Cart form

Collect extra information from shoppers on the cart page, like order notes, delivery dates, or a gift message, before they check out.

ℹ️ Before You Begin

Requirement	Detail
Plan	Premium or higher
Limit	One cart form per store
Cart page	Works with your theme's default cart page. A custom checkout flow or a cart template from a 3rd-party app may not work correctly.

📝 Step 1 — Create the Cart Form in the App

1. Open the app > click Forms > click Create new form button

2. The form template lists show up > select Oder > Cart form

3. In form configuration, add elements and configure the form based on your needs

4. Click Save to finish

📝 Step 2 — Add the Cart Form to Your Cart Page Template

⚠️ Special setup. This form type and template need extra care to work. After you create the form, contact us at [e-mail] we'll help you add it.

1. Form the Shopify admin page, go to Themes > click Edit theme

2. Go to the Cart page template

3. Click Add section > select Form Builder app

4. In the Form Builder app element, paste the embedded code of the cart form here

5. Click Save

6. Check the online store to view the result

📝 Step 3 — Check form on the site

1. Add a product to your cart and open the cart page

2. Look for the form above the Checkout button (or wherever you placed the block)

3. Fill it in and click Checkout

4. Open the order in Shopify and check Additional details

🧩 How the Cart Form Works

1 — Customer fills in the Cart Form

When shoppers are on the cart page, they see your cart form fields — for example:

• “How did you hear about us?”

• “Delivery date”

• “Message”

• “Agree to terms & conditions”

2 — The form is submitted when the shopper clicks Checkout

Unlike a normal form, customers do not press a “Submit” button. The form is automatically submitted the moment they click the Checkout button.

3 — You can view the Cart Form data inside the order

When the order is placed, the cart form answers appear inside the Additional Details section of the Shopify order page.

🛠️ Troubleshooting

Problem	What to check
Form doesn't show on the cart page	The form is in the Cart template, and app embed is on (Theme integration)
Still not showing	You may use a custom cart or 3rd-party cart app. Contact support.
Can't create a second cart form	Only one is allowed per store. Edit the existing one instead.
Answers missing from the order	The shopper must click Checkout from the cart page. Other routes skip the form.
No Submit button	This is normal. The form submits on Checkout.
